Tomoe's addiction to video games led her to asking the servants responsible for handling Chaldea's medical facilities to provide her with medical training just to play Surgeon Simulator. To their surprise, her dedication to simply becoming skilled at playing a mere video game ended up turning her into one of the most skilled medical practitioners in all of Chaldea with Nightingale and Asclepius deciding to make her a permanent member of Chaldea's medical staff. While hesitant at first, she had gladly accepted the offer as it would allow her to better serve her master and she was also offered video games as payment for her medical services.

Despite many members of Chaldea expressing shock initially, Tomoe is quite well liked as a member of Chaldea's medical staff due to her kind and motherly personality in addition to her medical skills. The child servants in particular seem to adore her quite a bit and is the only member of the medical team that does not frighten them. In addition, she has taken it upon herself to properly look after the health of her Master Ritsuka Fujimaru and demi-servant Mash Kyrielight, providing them with routine check ups as well as counseling support that they really appreciate.

After some practice using AI to generate art, I decided to create AI art of my favorite servant in Fate and I'm pretty happy with the results for the first batch that I generated. Also a huge shout out to Pixiv user Pig for suggesting the AI software to me along with some pointers. He makes some really good medical themed AI art and I'd highly recommend checking out his work.
